Surface Program Intake Form

Thank you for registering for a Surface Beach Volleyball private lesson or small group training session.

To help Coach Eric prepare for your athlete's training, please complete the Surface Program Intake Form before your scheduled session.

The information you provide allows us to:

• Learn about your athlete's volleyball experience and goals.

• Prepare drills and coaching specific to your athlete's needs.

• Collect parent, emergency contact, and medical information.

• Confirm important program policies and required acknowledgments.

The form takes approximately 5–10 minutes to complete.

**Please submit one intake form for each participating athlete.**

Waiver Info Needed

Liability and Possible Medical Waiver Info

The JVA waiver is required annually for each participant in our beach volleyball lessons. If you have already submitted a waiver, there's no need to worry.

To complete the intake form, please use the following information:

Club: Select Surface VBC (Surface Beach Volleyball Club) from the dropdown menu.

Director Info: Eric Poppelman, espoppelman@surfacevbc.com, 651-261-1483.

Team Name: List the first names of the girls participating in the volleyball training lesson or small group session.

**If your athlete has no medical needs or concerns, you can skip the medical portion. Just scroll to the bottom and hit submit.
